SKV Community Nursing was established to bring experienced, personalized nursing care into the home, helping people receive professional clinical support in a familiar and comfortable environment.
Led by an experienced Registered Nurse with more than 20 years across acute, emergency, neurological, medical, oncology, high-dependency, aeromedical and community care, SKV brings extensive clinical assessment experience and sound clinical judgement into the home.
Qualifications include a Bachelor of Nursing, Postgraduate Certificate in Advanced Nursing Practice and Graduate Diploma of Nursing specializing in Emergency Nursing.
Our approach is centered on safety, dignity, continuity of care and clear communications with clients, families, carers and other healthcare professionals.
RN-led nursing care at home
SKV Community Nursing services are tailored to your individual needs, care plan and circumstances. Care is provided within the Registered Nurse's scope of practice, with appropriate communication and escalation to your treating healthcare team when required.

Wound and pressure injury care
Professional wound assessment, dressing support and pressure injury prevention tailored to your clinical needs. We monitor progress and communicate concerns or changes promptly.
Post-hospital and post-operative support
RN-led follow-up after hospital discharge or surgery, including clinical review, recovery monitoring and practical education for clients, families and carers.
Clinical assessments and observations
Comprehensive nursing assessments and observations in the home, with clear documentation and escalation to the appropriate healthcare professional when needed.
Medication-related nursing support
Nursing support with prescribed medications within scope, including administration where appropriate, monitoring, education and communication of concerns or changes to the treating team.
Continence and catheter care
Respectful, private nursing support for continence needs and catheter care, including monitoring, routine catheter-related care, education and guidance for clients and carers.
Chronic and complex condition monitoring
Ongoing monitoring for chronic or complex health conditions, helping identify changes early and support coordinated care with your healthcare team.
Health education and self-management
Clear, practical education to help clients and carers understand care needs, recognise warning signs and feel more confident managing health at home.
Disability-related community nursing
Clinical nursing support for eligible self-managed and plan-managed NDIS participants, tailored to individual health needs and delivered within the Registered Nurse's scope of practice. SKV Community Nursing is an unregistered NDIS provider
Care coordination and communication
Collaborative communication with families, carers, GPs, hospitals and allied health professionals to support continuity, shared understanding and safer care.

Appointments & Availability
Scheduled mobile RN appointments are available from 8:00 am to 8:00 pm, 7 days a week, subject to availability. Same-day or short-notice visits may be available depending on location, clinical urgency and nurse availability.
Service Area
We provide mobile nursing across Pakenham, Officer, Beaconsfield, Berwick, Clyde, Cranbourne, Narre Warren, Koo Wee Rup, Emerald, Cockatoo, Gembrook, Garfield, Bunyip and surrounding communities across Cardinia, Casey and Melbourne’s south-east. Visits outside the standard area may be available by arrangement.
